What Is IASTM? The Soft Tissue Technique That Helps Athletes Heal
If you’ve ever walked into a sports rehab clinic and seen a clinician working on a patient with what looks like a stainless-steel butter knife, you’ve witnessed IASTM in action. It looks a little unusual at first glance — but the science behind it is solid, and the results speak for themselves.
IASTM stands for Instrument-Assisted Soft Tissue Mobilization. It’s a hands-on
treatment technique that uses specially designed tools to detect and treat restrictions in the soft tissues of the body: muscles, tendons, ligaments, and the connective tissue (fascia) that wraps around all of them. A Brief History
IASTM evolved from Gua Sha, a traditional East Asian healing practice that has been used for centuries to promote circulation and release tissue restrictions. Modern IASTM instruments are precision-engineered from stainless steel or other materials, designed to fit the contours of different body regions and provide tactile feedback to the clinician.
Techniques like Graston® and FAKTR are well-known commercial systems, though many highly trained clinicians use their own evidence-informed approaches.
How Does IASTM Work?
When soft tissue is damaged through injury, overuse, or surgery, the body lays down scar tissue and collagen as part of the healing process. This is normal and necessary — but when scar tissue accumulates in a disorganized pattern, or when fascial adhesions form between layers of tissue that should slide freely against each other, the result is restricted movement, pain, and reduced function.
IASTM instruments are applied to the skin with a small amount of lotion or gel, and the clinician uses specific strokes and pressure to:
• Detect areas of tissue restriction and scar tissue accumulation (they feel different under the instrument than healthy tissue)
• Break down disorganized scar tissue and fascial adhesions
• Stimulate a local inflammatory response that kicks off the healing cascade and promotes the remodeling of collagen into more functional tissue
• Improve local circulation and lymphatic drainage to the treated area
This process essentially gives the body a “reset signal” — telling it to remodel
dysfunctional tissue into healthier, more organized, and more functional tissue.
What Does IASTM Feel Like?
Most patients describe IASTM as feeling like a deep, focused massage with an
instrument. Some areas of restriction may feel tender during treatment — especially the first session — and mild redness (petechiae) may appear on the skin afterward. This is normal and resolves within 24–48 hours. Soreness similar to post-workout muscle soreness may occur for 1–2 days after treatment; this is a sign the healing process has been activated.
With each subsequent session, most patients report decreasing sensitivity in treated areas as the tissue quality improves.
What Conditions Does IASTM Treat?
IASTM is effective for a wide range of musculoskeletal conditions, including:
• Plantar fasciitis and heel pain
