Back to the Field: A Week-by-Week Guide to Safe Sports Return

Every athlete wants the same thing after an injury: to get back to their sport as fast as possible. And every parent wants the same thing: to make sure their kid goes back the right way, not just the fast way.

The tension between those two goals is where most return-to-sport decisions go wrong.

Pressure from coaches, teammates, and even the athlete’s own competitive drive can push a young athlete back into play before their body is truly ready — and the result is often a reinjury that’s worse than the original.

Why Returning Too Soon Is Dangerous

Reinjury rates are significantly higher in athletes who return before adequate healing and rehabilitation have occurred. For ACL tears specifically, studies show that athletes who return before meeting objective performance criteria are up to 15 times more likely to reinjure the same knee.

Beyond the physical risk, premature return can:

• Extend overall recovery time by months

• Lead to compensatory injuries in other areas (the opposite knee, hip, or ankle)

• Create fear-avoidance patterns that undermine long-term athletic confidence

• In the case of growth plate injuries, cause permanent damage if the injured area is loaded before it’s healed


The Phases of Return to Sport

A proper return-to-sport process is not a single moment — it’s a progression through distinct phases, each with its own objectives. Here’s how we approach it:

Phase 1: Protect and Reduce Inflammation (Days 1–7)

Immediately following injury, the goals are to protect the injured area, control pain and swelling, and maintain as much surrounding function as possible. Active rest (keeping the athlete moving in pain-free ways) is almost always better than complete rest. This phase includes soft tissue care, gentle joint work, and targeted exercises that don’t stress the injury site.

Phase 2: Restore Range of Motion and Basic Strength (Weeks 1–3)

Once acute inflammation has settled, the focus shifts to restoring full mobility to the injured joint and beginning progressive loading of the healing tissues. This is not the time to push hard — it’s the time to rebuild the foundation. Pain should remain minimal throughout this phase.

Phase 3: Rebuild Strength and Neuromuscular Control (Weeks 3–6+)

This phase involves progressive resistance training, single-leg stability work, and beginning sport-relevant movements. The athlete should be able to perform basic sport skills (running, cutting, jumping) without pain or compensation before advancing.

Objective strength testing (comparing the injured side to the uninjured side) is used to guide progression.

Phase 4: Sport-Specific Training (Weeks 6–10+)

Sport-specific drills, agility work, position-specific tasks, and progressive intensity loads are introduced. The athlete begins practicing at reduced intensity before working back to full-speed, full-contact situations. This phase requires close communication between the rehab team, the athlete, and the coaching staff.

Phase 5: Return to Full Competition

Full return to competition is appropriate only when the athlete has met objective performance criteria: symmetric strength, full range of motion, no pain with sport-specific activities, and — critically — psychological readiness. The last point is often overlooked but is highly predictive of reinjury risk.


The Psychological Side of Return to Sport

Fear of reinjury is real, and it’s one of the biggest predictors of reinjury after an ACL or significant ligament injury. Athletes who return to full competition before they feel psychologically ready — even if they’re physically cleared — tend to protect the injured limb unconsciously, which alters mechanics and increases injury risk.

We assess both physical and psychological readiness as part of our return-to-sport process. Open conversations between the athlete, parents, and the rehab team are essential.


A Note on Timelines

Every injury is different, and the timelines above are general frameworks. An ankle sprain may follow a 2–3 week progression. An ACL reconstruction typically requires 9–12 months of dedicated rehabilitation before full return to competitive sport. A growth plate injury may require a modified approach that prioritizes bone healing above all else.


Don’t compare your athlete’s timeline to their teammate’s or to something you read on the internet. Compare it to objective criteria set by their treating clinician — someone who knows the specific injury, the specific athlete, and the specific demands of their sport.
